2018 version issues

The 2018 version was missing the first four verses of Luke 1 and the first three verses of Revelation 1. This was fixed in the version released January 13, 2019, but if you purchased one of the earlier affected copies, I recommend printing out the following revised pages and inserting them into your copy. Apologies for the mistake.
